A chart fact is not documentation of today's visit
Admin pastes what is already on file. It becomes a green chip the doctor is asked to echo — not a gap, and not an assumption.
The doctor says one line instead of the biller writing an appeal
Everything already on file becomes a prompt in the room. The fact that would have been “in the chart somewhere” ends up in the note, where the payer reads it.
“It's in the chart” does not pay a claim
The x-ray report, the PT discharge summary, the prior injection date — a payer reviewing today's service reads today's note. Facts that live only in the chart are invisible to it.
The doctor is prompted, not second-guessed
Each on-file fact becomes one line to say out loud. That is the whole intervention, and it takes a sentence.
It separates two different failures
Missing from the chart is an operational problem. Present in the chart but absent from the note is a documentation problem, and only the second one is fixable in the room.
Try it on your own words
Paste a chart and a note, see what still needs saying. Leave it empty to run the sample case. Use made-up details — this is a public page, so do not paste real patient information.
